Welcome back to another wonderful year at Southwest Independent School District. Where it's always GAME ON! One Team. One Purpose. Every year the nurses strive to provide safe learning environment so students may succeed in their education. Vaccine Requirements for 2026-2027 School Year

To the Parents of all students turning 4 years old or entering 7th grade for the 2026-2027 school year. Just a friendly reminder that your future 7th grader will need two vaccines for the next school year. There are also several 4yr old vaccine shots required by the State of Texas. See the attached letter for more information. If you choose not​ to vaccinate your child, click on the button "Vaccine Opt Out Form for Reason of Conscience" to print out a PDF form to be filled out by a notary public.

Getting ready for the 2026-2027 school year. It is recommended that students with chronic conditions such as Diabetes, Asthma, Epilepsy, Anaphylactic Allergies, ect. get updated physician orders over prior to the 1st day of school. The mission of the SWISD Health Services Department is to guide students toward an optimal level of health, thereby enhancing their ability to obtain the maximum benefit from the educational experience. Having an up to date physician order and action plan will assist the school nurse in providing safe and effective care while your student is on campus. See below for Health Service forms.

Legislation from the 89th Texas Legislative Session passed in 2026 requires parents and guardians to provide annual consent for certain school-based health, counseling and social services along with permission for other activities. You may log in to Frontline to complete online forms. You may also utilize the following link to obtain the SWISD 2026-2027 form. Please fill out and return to your campus front office/nurse.

PARENTS AND GUARDIANS

Please send your students to school with an extra set of clothing. Providing extra clothing will be extremely helpful when students have personal hygiene accidents, spills, or damage to clothing.
