Please submit an original hard copy of your child’s Medical Eligibility Form to the nurse's office. We do not to accept Photocopies/Faxes/Emails.
If you have any questions regarding online registration or the activity fee, please contact the school’s Athletic Office. For questions related to the Health History Update or Medical Eligibility, please reach out directly to your school nurse.
