YRBS - Youth Risk Behavior Survey
Dear Berea-Midpark High School Parents/Guardians,
We are writing to inform you about an important opportunity for our students to participate in a nationwide survey that helps us improve school health and wellness programs: the Youth Risk Behavior Survey (YRBS).
What is the YRBS?
The YRBS is a voluntary, anonymous, and confidential survey developed by the Centers for Disease Control and Prevention (CDC). Please see the attached YRBS Fact Sheet.
The YRBS helps identify students’ health and safety habits so communities and schools can focus their efforts on making improvements where needed. Healthy lifestyles for students mean longer, more productive lives for young people, as well as improved learning in the classroom.
The survey will ask questions about:
Nutrition and physical activities.
Injuries and weapons.
Tobacco, alcohol, and other drug use.
Sexual health behaviors.
The results provide valuable, aggregated data that Berea Midpark High School and the district use to develop and enhance programs that better support the health, safety, and academic success of all our students.
Survey Logistics and Participation
The YRBS will be administered during school hours on Monday, November 24th, during 2nd period at Berea Midpark High School.
Your student's participation is completely voluntary and requires your written permission (passive or active consent/assent form). All responses are kept anonymous and confidential. No identifying information is collected, and students may skip any question they are uncomfortable answering.
The consent form is attached in the link below and a paper copy will also be sent home with your child. Please review this information carefully.
YRBS Opt In/Opt Out Consent Form - Electronic Form - https://forms.gle/ubnne1r2PH3uPsrV6
YRBS Opt-In/Opt-Out Form - Paper Form - Print and return to the main office.
Incentive for Returning the Form
To encourage a high rate of parental response and student participation, every Berea Midpark High School student who returns the YRBS consent form—regardless of whether you choose for them to participate or not—will receive a Dunkin' Donut Gift Card.
We strongly encourage you to review and return the form promptly to ensure our data accurately reflects the needs of our student body and to make sure your student receives their gift card.
If you have any questions regarding the YRBS and/ or would like to review the questions, please do not hesitate to contact Meredith Ondak, Coordinator of Behavioral Health and Wellness at mondak@bereaschools.org or 216-898-8300.
Thank you for your cooperation and commitment to the health and well-being of our students.
Sincerely,
The Berea Midpark High School Administration
