6th Grade Vaccination Requirements

In addition to previously required immunizations, all students entering 6th grade are required to receive one dose of the Tdap vaccine (tetanus, diptheria, pertussis) and one dose of the meningococcal vaccine (MCV4). Any student previously exempt from immunizations due to medical or religious reasons must sign a new exemption form (available in the office). 2026-2027 Health Forms

All health related forms are included in the electronic Annual Update that is required for all incoming and returning students. Electronic forms are available via Infinite Campus Parent Portal after mid July 2026 and must be completed before school resumes.

  • Health Services Website
  • Medical or mental health conditions
  • Current Medications
  • OTC (over-the-counter) Medication Permission
  • Permission for nurse to view immunization records through SD DOH

Self-Administration of Medication

Following updates to South Dakota legislation, our district has revised its policy regarding student self-administration of medication at school. Students who wish to self-carry and self-administer prescription medication must have:

  • A treatment plan signed by a licensed healthcare provider, and
  • A completed Self-Administration Authorization Form signed by a parent/guardian.

The Self-Administration Authorization Form, along with optional treatment/action plans, is available on the district website under the Health Services section. We encourage families to discuss these requirements with their healthcare provider over the summer and complete any necessary paperwork before the start of the 26-27 school year.

Please note that a signed healthcare provider treatment plan is only required for students who will self-carry and self-administer medication. Students may continue to store prescription and over-the-counter medications in the school office with a parent/guardian-signed Medication Authorization Form, which is also available on the district website.

Student Accident Insurance

The Spearfish School District is not responsible for any accidental injuries that occur to students while under the school’s supervision or participating in interscholastic sports.

Our School District has partnered with Student Assurance Services to give families the opportunity to purchase accident insurance for their student(s). When enrolled, this coverage provides financial assistance with your out-of-pocket medical expenses should your student sustain an accidental bodily injury.

Reasons to consider coverage for your student:

1. Your primary health insurance has a high deductible – copay – coinsurance.

2. No other insurance on your student.

3. Your student participates in a sport where unexpected injuries commonly occur.
