IEP- Secondary Transition Present Education Levels/Assessments

Did you know?

Secondary transition is the process of preparing students for life after they leave high school, including participation in post-secondary education or training, employment, and community living. The entire process is based on individual student’s needs, taking into account each student’s strengths, preferences, and interests.

To help facilitate a successful transition from high school, students should be given assessments, interest inventories, surveys, checklists, questionnaires, rating scales, person centered planning tools, and or preference and aptitude tests yearly to identify and support the students goals after high school. This data guides the activities the student will participate in or have exposure to help ensure the bridge to adult life is smooth.

Student/Teacher Spotlight

Stefan S. is a 20 year old Parkland student currently participating in the LCCC/Parkland Transition Program. He enjoys learning about campus landmarks and maps, while working with the mentors on other independent work skills. These skills include time management and navigating the campus on his own. “I feel like a responsible, independent adult,” he states. Stefan is currently participating in the CLIU-Work Based Learning Program as well working at Red Robin, Samuel Owens restaurant, and Weiss bakery. He enjoys the tasks and plans to use the skills after graduation to retain employment fixing appliances. Mr. York, Stefan's teacher, enjoys the incidental teaching that this program offers. Students are receiving their education alongside their same age peers in a community setting, outside of a high school. He can provide instruction using problem solving on topics that come up during the students’ daily interactions and experiences. We are setting them up for a successful transition into the adult world.
