Oct. 2 Exclusion Day

Dear Parents and Guardians,

This is a final reminder regarding mandatory student health compliance. Per state law and district policy, all enrolled students must have current physical examination records and up-to-date immunization documentation on file.

Effective Thursday morning, any student missing required medical documentation will be excluded from school and may not attend until compliance requirements are met.

If your child is currently non-compliant, they must remain home until the school receives one of the following:

• Completed Medical Records: Official physical exam forms and updated immunization records submitted to the school office.

• Proof of Appointment: Written confirmation from your healthcare provider’s office verifying a scheduled appointment for the required exams/vaccinations.

Families with outstanding documentation have previously received direct notifications from our staff requesting that documentation of appointment or the medical records be received no later than tomorrow.

If you have questions regarding your child’s health records or need to submit documentation before Thursday please contact the main office as soon as possible
