Greetings DeSales Families,
As we are approaching the start of the new school year, I would like to remind all DeSales parents to fill out and return the health history form and emergency health form (links below) and return to the health office as soon as possible. Please include new and existing medical conditions that we need to be aware of. This annually updated information ensures I have the most current medical information as well as the best way to contact you regarding your child or in the event of an emergency.
Medical orders from physicians for medications or medical accommodations at school must also be submitted yearly and dated for the current school year.
A physical for all students new to DeSales as well as for grades Pre-K, K, 1st, 3rd, 5th, and 7th dated within 12 months from the first day of school will also need to be provided.
These items can be faxed to 434-4002 Attn: Nurse.
