Transition Assessment
Welcome!
Transition Assessment is assessment that gathers information on the student's interests, abilities, and preferences in the areas of post-secondary education & training, employment, and independent living. Important things to keep in mind when deciding on which transition assessment to use...
- Use a variety of assessments- When a student interview is the sole assessment used, the student's opportunity to explore careers is limited. Remember, a student may not realize that they are talented or interested in a certain skill (i.e. working with their hands) if they are solely focused on one career path (i.e. veterinarian).
- Address all three areas (post-secondary education & training, employment, and independent living) in the Transition Section of the IEP using the data from your assessments.
- Postsecondary Goals are formulated using the data from the transition assessments.
- Use assessments to identify any gaps When a student is interested in a career path that may not be congruent with their abilities, assessments are a great way to showcase other areas that the student is demonstrating interests. We should always presume competence. However, if the goal is not realistic, the student doesn't want to attend further schooling, or the student does not want to move to pursue the career, then we need to match the student with another career choice that they have demonstrated interest in.
Transition Assessment & Activities Mapping Guides
To ensure that a variety of assessments are used and a coordinated set of activities are planned, using a transition assessment protocol is useful. Below, I have included several examples of these protocols/mapping guides. I would be happy to assist your district in creating your assessment protocol according to your needs.

Community Based Instruction
CBI is one of the most important tools in generalizing skills learned within the classroom. When executed with designated individual goals and data collection, the impact can last a lifetime. Below are a few of the rubrics I designed while I was in the classroom. Please feel free to use them/change them as you would like. If a student is struggling in a designated area, it is important to use a task analysis with the student to meet their goal.
Work-Based Learning
Work-Based Learning, in my opinion, is one of the greatest tools we can use with students to gather their interests, strengths, and needs. A student may think they want to work in a bakery until they stand on their feet all day. Likewise, a student may think they will hate working in a grocery store until they experience it. While in the classroom, I saw these reversals in thinking firsthand. It is crucial to get students out into the workplace to observe their skill levels and take data on progress. Below, you will find a few resources that can be used to take data on work skills. I also created a rubric for work-based learning while I was in the classroom. Just remember to follow the labor laws when placing students in work settings. If you have any questions pertaining to this, please contact me.
