Nurse Related Information
We wanted to share some important information with you for college. Included in the following information is a college readiness information sheet regarding vaccines that are needed for college. Please remember that the Meningitis vaccine that the college wants you to send is the MCV4 (serogroups A,C,W,Y) such as Menactra/Menveo vaccine, the other meningitis vaccine Meningococcal B serogroup (Men B/Bexsero) are highly suggested but not required by the college. This vaccine affords added protection from additional strains of Meningitis. If you would like additonal information about the Texas vaccine exemption form please go to this link for instructions https://www.dshs.texas.gov/immunization-unit/texas-school-child-care-facility-immunization/texas-immunization-exemptions
It is also suggested that you make sure you have completed your HPV vaccine series. Many of you have already completed it in middle school but some may have skipped it our not completed the whole series. Please check with your provider if your need a booster or have questions.
We also highly suggest that you complete the Imm Trac Adult consent form so that your vaccine record will be retained by the Texas ImmTrac vaccine registry for your lifetime. At the age of 18 your record leaves that system and is archived and not available to you. By completing this form your record will stay active in the system for your lifetime. You will need to send your completed form to the Comal County or Bexar County Health Dept to have your adult record activated in the system (do not return to the Pieper High School Clinic). This form may be accessed here- https://www.dshs.texas.gov/sites/default/files/immunize/immtrac/docs/F11-13366.pdf
