Dear Haskin Elementary School parent/guardian;
Our school is participating in a school climate survey conducted by the Center for the Study and Prevention of Violence (CSPV) at the University of Colorado. The purpose of this survey is to identify the strengths and needs of our school regarding school safety and violence prevention. The results will help us identify areas needing improvement and develop strategies to address those needs. In addition to surveying the students, we want to gain the perspective of all parents and guardians who have children who attend our school.
The survey is anonymous and is taken through a secure online server. Your participation is completely voluntary. The survey can be completed in approximately 15 minutes. Only one survey per family needs to be completed. Our goal is that at least 80% of households complete the survey. Response rates below 80% cannot be considered representative of the total population of parents and guardians.
It is very important that once you log in to the survey that you complete it in one sitting. Logging out of the survey will end the session, and you will not be able to log back in to the same survey in order to complete it.
To participate in the survey, go to the following link:
https://www.safe-schools.org/school-climate/parent/?b=501819-90002
If you do not have access to a computer to complete the survey, please let me or Katrina Ruggles know and we will make arrangements to give you access to one of the school’s computers. If you need to take the survey in Spanish, you will see links in the upper right hand corner of the survey page for both English and Spanish.
You will have approximately two weeks to complete the survey. After all the data has been collected, researchers at CSPV will analyze the data and issue a summary report.
Thank you for your participation in this important survey!
Sincerely,
Katrina Ruggles, Center School District

University of Colorado’s Center for the Study and Prevention of Violence SAFE COMMUNITIES ~ SAFE SCHOOLS ONLINE SCHOOL CLIMATE SURVEY Consent Form
Dear Parent/Guardian,
We are asking permission for your student to participate in an online survey that will be administered during the 2022-2023 school year. All students in grades 3-5 are being invited to participate.
The purpose of the survey is to investigate the conditions that exist at your student’s school. Topics include bullying and violence at school; the relationships children have with their parents and teachers; and drug, alcohol or cigarette use by students and their peers. You can see a copy of your student’s survey here:
Test site link: SCSS Climate Survey Parent Preview Link Enter code: 501819
Once you access the test site and enter the school code, select the “Students” survey and click on the ‘continue’ button. You will need to click on “I assent” at the bottom of the page to access the survey. You do not need to enter any data, just click through each page to view the items included on the survey.
The survey will be conducted anonymously. Any and all research information that is collected will be protected against all forms of disclosure, except as required by court order. The identities of all survey participants will not be recorded, and any information that could lead to the identification of individual students completing the survey will be removed.
This survey has no direct risks or benefits to your student. It is hoped that this survey and related research efforts will provide benefits to schools in the form of improved knowledge on the origins, dynamics, and impacts of school violence and on ways to make our schools safer in the face of increased violence.
Research indicates that when schools experience violence, substance use and bullying, learning is hampered, academic performance declines, absenteeism increases and the general well-being of both children and teachers is diminished. This survey allows your student’s school the opportunity to identify its strengths and challenges, and to address problem areas, which could lead to increased academic performance as well as improved well-being for students and staff.
Your student’s participation in this survey is voluntary and without compensation, and he or she may refuse to answer any specific question in the survey. If your student wishes to withdraw from the survey, he or she may do so at any time without giving any reason or explanation. If he or she chooses to withdraw from the survey, any information he or she may have provided prior to withdrawing will not be used.
You have the right to decline your student’s participation in the survey if you wish. If you do decline, your student will be allowed to read, complete school assignments or participate in an alternative activity while the survey is being administered.
The survey is being conducted by our school through an agreement with the Center for the Study and Prevention of Violence (CSPV) at the University of Colorado, Boulder. If you have any questions regarding the survey, please feel free to contact Sabrina Arredondo Mattson, PhD at 303-735-1633.
Please check the box below if you DO NOT want your student to participate in the survey and send the letter back to school.

Miss. Hannah, 2ND AND 4TH GRADE
Art Culture Project for 2nd and 4th Grade
Over the next couple of months, the 2nd and 4th graders will be exploring our own cultures. In order to do this, we must determine where we all came from. Many families may originate from the same country, but not the same city or region. It is the regions we will be specifically looking at later during art class. When it is time for the students to start their project, they will be given a worksheet which will help to guide them in researching their culture. They will need to bring that worksheet home with them and have their parent or guardian sign it so that I know they talked with them about it.
The goal of this assignment is to investigate the art from the region their family is from during art class. If their family has been in America for many generations, I want them to keep looking back until they find family born outside of the United States. I want this assignment to be a conversation with their family about their history. The only part I need to be told is where the family originated, not who the first person to come to the United States was. I really want the students to be able to connect with a specific city or region, but if it cannot be found, the closest you can find is appreciated.
Please do not fill out the form for them, but if you wish to participate in the research, you may help them. The questions can be researched during class, but I need them to know which city or region prior to class time.
If you have any questions or concerns, please contact me via email: hmetzger@center.k12.co.us. If you would like to speak over the phone or in person that can be set up via email.
Thank You,
Miss. Hannah
