Caring for Students
Documenting Support for General Education Students
Plan of Care Development for Students Who Receive Behavioral/Medical/Mental Health Services
Students who receive behavioral, medical and/or mental health care during their school day must have a written Plan of Care that is reviewed and updated annually. Plans must be developed for every student you are treating, regardless of their Medicaid eligibility status. However, you will bill for your general education students who have a written Plan of Care and are found to be Medicaid eligible.
Staff who are expected to participate in the MDHHS expanded school services are staff who regularly perform medically necessary services for students who qualify for therapy/treatment/counseling services outside of IDEA regulations:
- Board Certified Behavioral Analyst
- Nurses
- Professional Counselors
- Psychologists
- Social Workers and,
- Paraprofessionals/Personal Care Aides.
Your district or academy administrator has identified you as a staff member who provides care and support to students who do not receive Special Education Services. In order to help support your efforts with developing written documentation for General Education students, there is a Caring for Students Plan of Care that is available for you to use in order to document and track the work that you are already doing.
In this Special Edition of the Medicaid Messenger, we are including:
- a fillable PDF Plan of Care* for those who do not have access to MISTAR Forms
- an instruction sheet on how to complete the fillable PDF Plan of Care
- a click-by-click guide for staff who have full MISTAR access for completing the Plan of Care in MISTAR Program Forms
- a PDF of the Caring for Students - Frequently Asked Questions sheet
*Schools who should be using the attached fillable C4S Plan of Care include: David Ellis Academy West, Summit Academy North and Trillium Academy. All other districts and academies are full MISTAR users and should now be accessing the C4S Plan of Care available in MISTAR>Program Forms.
