Students in middle and high school - Please make sure your student(s) sports physicals are up to date, as they expire yearly. The physical will need to be current for students to attend the official first day of practice of any school sport. Sports starting in August are cross country, football and volleyball.
The Mancos Health Clinic will be doing sports physicals. We will keep you posted on upcoming dates.
Blue Cards
To begin a sport in middle school or high school you must obtain a Blue Card prior for each sport. All paperwork must be completed yearly and each sport requires the athletic fee of $100 for high school and $50 for middle school. Blue Card requirements and all forms are available on the website or from the athletic office.
Medical and Health Information
Summer is a great time to make sure your student(s) is/are up to date on vaccinations. This letter from the Colorado Department of Health and Environment (CDPHE), lists what is required for school entry. Please keep in mind that your student(s) may be excluded from school if records are not up to date.
If your student(s) is/are exempt from vaccinations, please visit the CDPHE and complete the medical or nonmedical exemption forms. Nonmedical exemptions expire June 30th each year, therefore this form needs to be completed after July 1st for it to be valid for the upcoming school year. Alternatively, for nonmedical exemptions, you can complete a course online (approx 20 min. video) to obtain the certificate.
If your student(s) needs a meal modification, the ‘Medical Statement for Meal Modification’ needs to be completed each year and signed by your family physician.
If your student(s) has asthma and may need an inhaler at school and/or will self carry, please make sure the Colorado Asthma Care Plan is completed and given to our school nurse. If your student self carries, the school requires the asthma care plan and the asthma self carry contract, which is attached.
If your student has severe allergies that can result in anaphylaxis, please have your medical provider complete this Allergy and Anaphylaxis Care Plan
If routine/prescription medications are stored at school and to be administered by staff, please complete the Permission for Medication form. Also, if your student is ever in need of antibiotics (or any type of medication, prescription or over the counter) to be administered at school, please contact the School Nurse. All medications need to be in their original container. The school nurse does have a list of common over the counter medications that parents give permission/do not give permission to administer. There is a doctor's standing order for these medications. Otherwise, any other type of medication must have this completed form. This also applies for overnight field trips. For all medications currently being administered by the school nurse and for those kept at school for emergency use, a form must be completed yearly.
Transportation
Bus Routes
Once registration is complete, bus routes will be confirmed. If your student rides the bus, the transportation department will contact you by Friday, August 8th to confirm your drop off and pick up times.
Meals
Breakfast and lunch will be available to all students for FREE, through the Healthy School Meals for All program and our participation in the Community Eligibility Provision (CEP).
Help us Secure Funding for Mancos Schools!
As always, we encourage all families to fill out the Family Economic Data Survey (FEDS) form during registration—even if you think you may not qualify. This form has replaced the old Free and Reduced-Price Meal application and is a key part of our participation in the Community Eligibility Provision (CEP). It helps secure vital funding for our schools and supports programs that benefit all students.
