Please provide an update for the people responsible for Discipline Data: School Discipline Administrators 2022-2023 School Year

It is imperative to enter address changes as soon as they are received. This is due to required updates from ODE for demographic data for Parents to utilize available resources.

• Student Change of Information Form

• CPS Proof of Residency Form

For P-EBT questions/confirmation, please send the student's name, date of birth and student ID number to Ebony Johnson. If the student is not listed, we will research and get back to you.

If the student is eligible, the Parent or Guardian will then need to follow up with Job and Family Services. For questions regarding P-EBT, Families will need to contact the P-EBT customer service line at 1-866-244-0071.
