Athlete Registration using FINALFORMS
We are excited to announce that the Haslett Public Schools Athletic Department has partnered with FinalForms, an online forms and data management system. FinalForms allows you to complete and sign athletic participation forms for your student online. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice. You will review your information once per school year to verify information is current and sign the forms.
All Viking student-athletes, Middle School and High School, are required to be registered in our online athletic program. To register your student-athlete, please go to: https://haslett-mi.finalforms.com/
A couple of hints:
Bookmark the site or save it to your desktop on your mobile device or computer (Do not Google search for it in the future as you will not go directly to the Haslett site.)
Have required information available, such as medical history/health information, insurance company policy numbers, doctor, dentist and hospital information. This will make the registration time more efficient.
Parents will need to fill in your student’s email address. Finalforms requires that the student’s @haslett.k12.mi.us email address be used. Obtain this from your son/daughter. Your student will then need to check their email for information on how to complete their portion of the registration.
Please note, once you register, you will receive an email to confirm your account, create a password, and then complete the required forms.
Before the beginning of a new school year, either you or your student should click on the sports they are interested in. They do not automatically roll over. If a coach sends out information prior to the season about off-season work-outs or opportunities, your student-athlete will not get the emails unless they have chosen that sport for the new year.
All forms will need to be completed in order to practice on day one. Please take care of this as soon as possible in case you run into any setbacks. Physicals can be uploaded for your convenience. On-line payments can also be accepted on FinalForms if your family chooses to use this feature, if not, we will still accept cash and checks.
