Online Registration Through Final Forms
The athletic packet is available to fill out online through Final Forms. Once you have a user account you can use the same account each school year to register for that school year’s athletic opportunities.
Sports Physicals
Sports physicals are valid for two years from the date they were conducted unless noted by a doctor that the Sports physical is good for one year. During a well child visit you can ask your provider for the sports physical clearance and they can provide it for you at that time. Most medical providers and walk-in clinics provide this service but it is helpful to ask when scheduling the appointment and booking in advance as the appointment slots tend to fill up quickly (especially in the fall).
Fees
Payment Information
Fees can be paid at the MMS office or online through your Family Access payment portal or by following the QR code. Please note, per district policy, we are not longer able to accept payments over the phone.
Sports Seasons
Season 1 (September): Baseball, Fastpitch, Track (Season will start the second day of school)

Team Placement
To maximize the experience for athletes, both individually and as a team, students will be assessed during the first week of practice and placed on a team to best fit their skill level: Varsity, Junior Varsity, C-Team or E- Team. Varsity, Junior Varsity, and C-Team will travel to other schools to compete. E-Team athletes will not travel, but will instead participate in an intra-mural type program that will focus on skill building.
Team placements will be communicated after the first week of practice. If a student athlete is not placed on a traveling team they will have the opportunity to participate in the E team program.

Baseball
Hello Wolves!
Baseball practice/ tryouts start on Wednesday September 4th at 2:30 PM to 5PM !!!
The first week we will be practicing/tryouts at 2:30 Wednesday through Friday (no weekends). We will pick back up Monday 9th through Friday the 13th. These will be 2:30 to 5PM as well. I will get you the game schedule and future practices schedule after the first week or so. The baseball season runs from the 2nd day of school until usually the 3rd or 4th week of October. There are no cuts. Everyone will be on a team and play baseball.
What your player needs to get done: Your player will not be allowed to practice/tryout until they have been cleared on Final Forms and cleared by our Athletic Coordinator Dave Rumpza. Here is the link for Final Forms. https://sumner-wa.finalforms.com/
Equipment - Baseball gear can be brought to school and stored in the boys locker room cage in the morning of Wednesday September 4th.and picked up after school in the same spot and the boys can change. Can do this daily if needed.
Baseball gear needed for practice and games-
For practice, players need to wear baseball pants,( any color) (no shorts) a baseball belt, cleats, a baseball hat and an athletic cup.
For games, players need to wear all of the above but need a pair of white pants for Varsity. JV/C a pair of gray baseball pants, black or navy belt and a Mountain View baseball hat. If you don’t have a MV hat you can order from our team store. (Link Below) If you think your player might make Varsity I would recommend getting both a pair of gray and a pair of white pants.
We do not supply gloves or cleats. Your player will need their own. We do have some team batting helmets and bats for those players that don’t have their own that they can use.
Bats - If you are purchasing your own bat or already have one, here are the bat regulations for our Plateau league. Can’t tell you exactly what bat your player needs at this moment since teams will be formed next week. But here is the info. This is the rule from last year and at this point it should be the same. Rule 4.15 Varsity players use BBCOR (-3) or USA. JV players use USA or USSSA (-5)
Game uniforms - The school provides the tops. You must have your own baseball pants, and belt. If your player likes to wear the knicker style baseball pants or likes to roll them up then I ask that they have a pair of black baseball socks showing.
The MV hat is purchased by you from our team store.
Team store - For every item sold we get $5 back to the baseball program. Middle School does not have boosters so the $ we get back is huge to help purchase things for the baseball program. (Store will close on Sunday 9/8. Turn around is about 2 weeks.)
The only item we ask you to buy is the one size fits all MV baseball hat for your player.
Optional items - There are three colors of lighter MV practice shirts for the players or parents if they want them. We also have a couple of hoodies available there as well.

Communication - We will communicate to families and players via Final Forms emails. So please check and make sure you have an email you check on Final Forms and enable your notifications.
Thank you and see you in the field!
