Senior Checkout and Final Transcript Request - Mandatory

Seniors are asked to complete mandatory tasks in order to complete their senior check-out from the Counseling Office. The Senior Checkout and Final Transcript form, found below, verifies that these tasks are completed, provides direction for transcripts, and provides important information required by the district. The mandatory tasks include:

  1. Complete the Senior Checkout and Final Transcript Form by May 1, 2026, and submit to Bridge West. This form lets counselors know where and how to send the senior's final transcript, including NCAA and NAIA. In addition, the form provides directions on how to request a final transcript for non-Common App colleges, NCAA, and NAIA.

  2. Complete the Senior-Year End Activities in the Schoolinks App, which is available in Classlink. Directions are provided in the Checkout Form.

  3. Complete the Onboarding Activity
    • On the Dashboard/Home Page, look for "To Do List", open the "Due Later This Year Tab", and click on "Senior Year End Activity". Complete the survey and answer all questions, then hit submit.
    • Upload proof of college acceptances and also any scholarships that have been offered (do not include HOPE/Zell). Full directions on the Senior Checkout Form.

Visit Bridge West if you need help or see a counselor in the cafeteria during both lunch periods.

Important Reminder

🎓 Seniors who do not complete the steps listed on the Senior Checkout and Final Transcript Request form will have their graduation tickets held until they are completed.

Meningococcal Vaccine Requirement

Has your child turned 16? If so, have they had the MCV4 booster shot?

This is a requirement by the GA Department of Health and Fulton County Schools for public school students. All students who are 16 and entering 11th grade must have an updated GA Form 3231 Immunization Form showing receipt of the booster vaccination. Once your student turns 16, you have 30 days to get the shot and update your form. Go ahead and get your shot now, so you don't have issues when school starts in August. Here is the link for the FCS flyer regarding the booster requirement: Immunization Requirements
