SAFE COMMUNITIES ~ SAFE SCHOOLS ONLINE SCHOOL CLIMATE SURVEY
Consent Form
Dear Parent/Guardian,
We are asking permission for your student to participate in an online survey that will be administered during the 2025-2026 school year. All students in grades 9-12 are being invited to participate.
The purpose of the survey is to investigate the conditions that exist at your student’s school. Topics include bullying and violence at school; the relationships children have with their parents and teachers; and drug, alcohol or cigarette use by students and their peers. You can see a copy of your student’s survey here:
Test site link: https://www.safe-schools.org/survey-326742 Enter code: 639736
Once you access the test site and enter the school code, select the “Students” survey and click on the ‘continue’ button. You will need to click on “I assent” at the bottom of the page to access the survey. You do not need to enter any data, just click through each page to view the items included on the survey.
The survey will be conducted anonymously. Any and all research information that is collected will be protected against all forms of disclosure, except as required by court order. The identities of all survey participants will not be recorded, and any information that could lead to the identification of individual students completing the survey will be removed.
This survey has no direct risks or benefits to your student. It is hoped that this survey and related research efforts will provide benefits to schools in the form of improved knowledge on the origins, dynamics, and impacts of school violence and on ways to make our schools safer in the face of increased violence.
Research indicates that when schools experience violence, substance use and bullying, learning is hampered, academic performance declines, absenteeism increases and the general well-being of both children and teachers is diminished. This survey allows your student’s school the opportunity to identify its strengths and challenges, and to address problem areas, which could lead to increased academic performance as well as improved well-being for students and staff.
Your student’s participation in this survey is voluntary and without compensation, and he or she may refuse to answer any specific question in the survey. If your student wishes to withdraw from the survey, he or she may do so at any time without giving any reason or explanation. If he or she chooses to withdraw from the survey, any information he or she may have provided prior to withdrawing will not be used.
You have the right to decline your student’s participation in the survey if you wish. If you do decline, your student will be allowed to read, complete school assignments or participate in an alternative activity while the survey is being administered.
The survey is being conducted by our school through an agreement with the Center for the Study and Prevention of Violence (CSPV) at the University of Colorado, Boulder. If you have any questions regarding the survey, please feel free to contact Sabrina Arredondo Mattson, PhD at 303-735-1633.
Please check the box below if you DO NOT want your student to participate in the survey and send the letter back to school.

BULLYING INFORMATION
Bullying is when someone repeatedly hurts, scares, or threatens another person on purpose. It can happen in many ways, like:
Physical bullying – hitting, pushing, or taking someone’s things.
Verbal bullying – name-calling, teasing, or saying mean things.
Social or relational bullying – spreading rumors, excluding someone, or embarrassing them in front of others.
Cyberbullying – using phones, social media, or the internet to say mean things or hurt someone.
The key part is that it’s intentional and happens more than once, making the person feel unsafe or unhappy.
Warning Signs of Bullying |
Unexplained injuries, lost belongings, sudden changes in mood or behavior, and avoidance of school.
What to Do
Tell a trusted adult (teacher, counselor, parent)
Don’t retaliate or bully back
Support the person being bullied
Keep evidence if online
Remember
Bullying is never the victim’s fault. Everyone deserves respect and safety.

EMERGENCY PROTOCOL
Keeping Our Schools Safe
At Center Schools, student safety is always our top priority. Like most schools across the country, we use the Standard Response Protocol (SRP).
This is a simple and clear plan that helps everyone—students, staff, and families—know what to do in different situations.
We want you, as parents, to feel confident knowing that your child is learning and practicing these safety steps throughout the year.
✅ The 5 SRP Actions
Hold – Students stay in their classroom while hallways are kept clear.
Secure – Everyone comes inside; learning continues as normal.
Lockdown – Doors locked, lights out, students stay quiet and out of sight.
Evacuate – Students move safely to another location (like during a fire drill).
Shelter – Students take safety steps for severe weather or hazards.
By practicing these steps, students and staff are prepared and ready—helping us create a safe place for learning every day.

Starting September 8th the District is initiating an automated system that will send email, texts, and Infinite Campus Inbox messages regarding your students attendance. When a student is marked absent in their class you should receive this automated message. Our goal is to communicate with families regarding their child's attendance in a timely manner. At the Secondary level (Center High and Skogland Middle) if a student is marked with an unexcused tardy a message will be sent starting at 4:20 PM.
If you have questions or concerns regarding who is being contacted please contact the office secretary or make the changes in your Campus profile.
Attendance Procedures 2025-2026

Connecting Families. Supporting Students. Building Success Together.
Connecting Families. Supporting Students. Building Success Together.
Dear Families,
My name is Dalia Molina, and I am honored to serve as your Middle and High School Parent Engagement Coordinator. My goal is to build strong partnerships between home and school so that every student has the support they need to succeed.
Parent engagement is more than attending events — it is about communication, collaboration, and connection. When families and schools work together, students achieve more academically, socially, and emotionally.
Throughout the year, I will:
Provide helpful resources and parenting support
Host parent workshops and family nights
Connect families with community services
Support communication between parents and school staff
Encourage family involvement in school activities
You are your child’s first and most important teacher. Your voice, your involvement, and your partnership matter.
If you ever have questions, need support, or want to get involved, please reach out. I look forward to working together to make this a successful school year for our students and families.
Together, we are stronger.
Warmly,
Dalia Molina
