Dear Parent/Guardian,

The Westfield Parent Back-to-School Packet is now available on the Genesis Parent Portal. This

electronic packet includes forms required for students enrolled in the Westfield Public Schools.

 Parent/Guardian Acknowledgement/Consent/Medical Form

 Pre-K - Grade 3 After School Dismissal Form

We ask that you carefully review and complete this Back-to-School packet. Please note that you must complete and submit these electronic forms before gaining access to your child(ren)’s class

assignment/schedule, which will be available on the Genesis Parent Portal.

Once you have submitted the Back-to-School packet and your child’s class assignment/schedule is posted, you will be able to view it in the portal (Mon, Aug 24 @ 10 AM).

Please login to https://parents.westfieldnjk12.org to update your contact information and to complete the Parent/Guardian Acknowledgement/Consent/Medical Form for each student. The electronic packet of forms must be completed for EACH STUDENT enrolled in the district and electronically submitted by one parent (guardian 1). Parents of students in Pre-K through Grade 3 must also complete an additional Pre-K - Grade 3 After School Dismissal Form.

Important things to remember:

 Click on the Contact tab to update contact information as soon as possible.

 All parents must complete the required Parent/Guardian Acknowledgement/Consent/Medical

Forms.

 The Pre-K through Grade 3 After-School Dismissal Form will only be accessible to parents of

students in those grades.

 You will receive an email once class assignments/schedules are available on the Genesis Parent

Portal.

• Schedules for the high school will be available on Monday, August 24 @ 3 p.m.
• Intermediate school schedules will be available on Friday, August 21 @ 3 p.m.
• Homeroom assignments for the elementary schools and Lincoln Early Childhood Center will be available on Monday, August 24 @ 10 a.m.

Additional instructions for all parents:

 Complete the "Parent/Guardian Approval" section at the end of the electronic packet to indicate

that you have read and provided the necessary information.

 Please be sure to click the "Update Answers" button at the end of the form. A dialog box will

appear with final instructions. Please note that you will be prompted to click “Update Answers” a

second time to complete your submission.
