Free Dental Care for All Students at Forest on Monday, April 27
Who: Preferred Dentistry Associates, P.C. (located in Evanston) is coming to Forest Elementary School to provide FREE DENTAL CARE for ANY STUDENT who wishes to participate. It doesn't matter whether you have dental insurance or not, ALL STUDENTS QUALIFY.
What: FREE DENTAL CARE includes: an exam, cleaning, fluoride treatment and dental sealants (if applicable).
When: Monday, April 27, 2026 - during the normal school day.
Where: Forest Elementary School
Details: FOR YOUR CHILD TO PARTICIPATE IN THIS PROGRAM, PLEASE COMPLETE THE ATTACHED PERMISSION SLIP AND RETURN TO SCHOOL BY Monday, April 20, 2026.
The State of Illinois requires that students in Kindergarten, 2nd, and 6th grades have a dental exam. The dental exam would fulfill that requirement. Parents will receive Proof of School Dental Examination Form for their child's exam and a copy will be put in their school Health folder.
