Sports Registration 2026-2027

All participating athletes will need to completely watch the online Orientation video and review Chapter X in the Family School Handbook with his/her parent/guardian.

• Mandatory for athletes in grades 7-12
• Baseline results valid for two from test date
• Contact Athletic Director for next ImPact Baseline Test Date

• HI DOE Physical for Athletics
• HI DOE Concussion Management and Study Form (grades 7-12)
• Pac-5 Student Participation Form (Pac-5 athletes)

If you prefer, the form will have the option to upload digital or scanned copies of the completed forms. They can also be turned in to the school office.

For Students in grades 6-12th that would like to earn P.E. credit through playing a sport, the P.E. Waiver request and agreement is at the end of this registration form.
